rooted in buddhism, mindfulness meditation has developed a prominent perch in the self-help movement. its popularity has been fueled by research that indicates mindfulness often reduces stress and promotes emotional well-being.
background distress and burnout among medical and psychology professionals are commonly reported and have implications for the quality of patient care delivered. already in the course of university studies, medicine and psychology students report mental distress and low life satisfaction. there is a need for interventions that promote better coping skills in students in order to prevent distress and future burnout. this study examines the effect of a seven-week mindfulness-based stress reduction (mbsr) programme on mental distress, study stress, burnout, subjective well-being, and mindfulness of medical and psychology students. methods a total of 288 students (mean age = 23 years, 76% female) from the university of oslo and the university of tromsø were randomly allocated to an intervention or control group. the control group continued with their standard university courses and received no intervention. participants were evaluated using self-reported measures both before and after the intervention. these were: the ‘general health questionnaire, maslach burnout inventory student version, perceived medical school stress, subjective well-being, and five facet mindfulness questionnaire’ and additional indices of compliance. results following the intervention, a moderate effect on mental distress (hedges’g 0.65, ci = .41, .88), and a small effect on both subjective well-being (hedges’g 0.40, ci = .27, .63) and the mindfulness facet ‘non-reacting’ (hedges’g 0.33, ci = .10, .56) were found in the intervention group compared with the control group. a higher level of programme attendance and reported mindfulness exercises predicted these changes. significant effects were only found for female students who additionally reported reduced study stress and an increase in the mindfulness facet ‘non-judging’. gender specific effects of participation in the mbsr programme have not previously been reported, and gender differences in the present study are discussed. conclusion female medical and psychology students experienced significant positive improvements in mental distress, study stress, subjective well-being and mindfulness after participating in the mbsr programme. trial registration nct00892138

attention is critical for successful performance in demanding real-world situations. yet, protracted periods of high demand may compromise attention and increase off-task thinking. herein, we investigate if mindfulness training (mt) may promote cognitive resilience by curbing attentional lapses in high-stress cohorts. two military cohorts were recruited during their high-stress predeployment interval. mindfulness-based mind fitness training (mmft)® was provided to one group (mt, n = 31) but not the other group (military control group, mc, n = 24). the mt group attended an 8-week mmft® course and logged the amount of out-of-class time spent practicing formal mt exercises. the sustained attention to response task (sart) was used to index objective attentional performance and subjective ratings of mind wandering before (t1) and after (t2) the mt course. in the mt group, changes in sart measures correlated with the amount of time spent engaging in mt homework practice, with greater objective performance benefits (indexed by a′, a sensitivity measure), and reduced subjective reports of mind wandering over time in those who engaged in high practice vs. low practice. performance measures in the low practice and mc groups significantly declined from t1 to t2. in contrast, the high practice group remained stable over time. these results suggest that engaging in sufficient mt practice may protect against attentional lapses over high-demand intervals. based on these results, we argue that mt programs emphasizing greater engagement in mindfulness practice should be further investigated as a route by which to build cognitive resilience in high-stress cohorts.

mbsr is an eight-week evidence-based program that offers intensive mindfulness training to assist people with stress, anxiety, depression and pain. mbsr uses a combination of mind-fulness meditation, body awareness, yoga, and exploration of patterns of behavior, thinking, feeling and action.
although mindfulness meditation is known to provide a wealth of psychological benefits, the neural mechanisms involved in these effects remain to be well characterized. a central question is whether the observed benefits of mindfulness training derive from specific changes in the structural brain connectome that do not result from alternative forms of experimental intervention. measures of whole-brain and node-level structural connectome changes induced by mindfulness training were compared with those induced by cognitive and physical fitness training within a large, multi-group intervention protocol (n = 86). whole-brain analyses examined global graph-theoretical changes in structural network topology. a hypothesis-driven approach was taken to investigate connectivity changes within the insula, which was predicted here to mediate interoceptive awareness skills that have been shown to improve through mindfulness training. no global changes were observed in whole-brain network topology. however, node-level results confirmed a priori hypotheses, demonstrating significant increases in mean connection strength in right insula across all of its connections. present findings suggest that mindfulness strengthens interoception, operationalized here as the mean insula connection strength within the overall connectome. this finding further elucidates the neural mechanisms of mindfulness meditation and motivates new perspectives about the unique benefits of mindfulness training compared to contemporary cognitive and physical fitness interventions.
